My recent was a few weeks ago in Vegas, won in the casino but dropped a $100 chip on the street and it rolled right into a storm drain.

I wasn't paying attention and bought the wrong size trash bags (144 bags) that are too small for my trash can in the kitchen.
I'll attempt to return to the club (no receipt). if they won't give me store credit I'll just give to the animal shelter - hopefully they can use them.

How about a near loss that happened in a way similar to yours?
I got out of the passenger side of a car and as I stood up, my wallet slipped out of the pocket of my nylon windbreaker jacket. I saw it glide over some oak leaves in the gutter and straight into the storm drain! Agh! I got down and saw the wallet had stopped on a leaf-covered ledge inside the drain but I could not reach it due to the bend in the opening.
The sun was beginning to set so I went home for a flashlight and spouse. We dashed to Walgreens where we bought one of those long-handled grabber things that people who cannot bend might use. Went back and I held the flashlight, DH did the "fishing" and the wallet was retrieved.I don't usually carry much more than ID and perhaps one CC or one DC. But at the time I had $300 cash, three CC, one DC, driver's license, and a small medical item.

A few month ago I was watching my buddy play pool at a bar and he was talking all kinds of smack so when it got to the end of one of the games and he had a medium difficulty shot to win I laid a hundo on the table and bet against him making it. I didn't care about the money, I just wanted to rag on him for missing a high stakes shot in front of a lot of friends.

On an early Saturday morning, I was walking back to my car from the farmer's market. Had my keys swinging in my hand. My car key actually fell off my key chain into a storm drain (I didn't drop the whole thing - just the one key). It couldn't have happened five seconds earlier or later but right over the drain. Luckily, I had the valet key in my purse so I could still drive home. I look in that drain every time I walk past it.

Wow, surprising to see so many things dropping into storm drains. I priced a duplicate key for my car and it's $90, cheaper than the $160 dealership but still too muchretired in 2009 at the age of 39 with less than 300K total net worth

It has been a few years (2013), but the biggest financial mistake I've made is with my retirement account. I was a new hire and I opted to not do anything for retirement.
I was 4 months pregnant and figured I'd only work a couple of years (ended up being 18 months) before becoming a stay at home mom. I thought that the little money that could have been put into the retirement account would have been eaten up by fees before retirement since the account would sit dead for so long. I simplified by only having my husband save for retirement.
Consequentially, almost $5,000 dollars in company matched money was left on the table, as well as tons of gains in the stock market. To add insult to injury, I am now researching opening an account with the very same company that I would have already had an account with.

I lost over $200K in my 401K once.
I had $450K invested in Vanguard Energy and Blackrock Natural Resources through my company's 401K.
In the valley of an inevitable correction, when the total value of these had sunk to $250,000, our company decided to change 401K administrators. It was a quid pro quo with a local bank and trust company: The bank's CEO was golfing buddies with the hospital CEO.
A deal was made whereby the hospital would get cheap financing for a big project from the bank, and in return, the hospital would award the 401K program to them.
That all sounds neat, but they came in and only offered us about 15 funds, and Vanguard Energy and Blackrock Natural Resources weren't among them.
So in the transition, they jettison'd me right out of those funds, with me taking a $200K hit.
I never contributed another dime to my 401K after that day, and made the decision that I would be leaving corporate america as soon as I was able.

Mine is relatively minor, but a major PITA. I was cooking spaghetti sauce for 20 people for my daughter's birthday party. I took a taste while cooking and it had an overwhelming salty taste even though I didn't add any salt. I had to throw it away, which was 3 Costco cans of tomato sauce, plus the sausage, onion, paste, and herbs. Then I ran out to Costco, repurchased everything and started again, only to find the salty taste again. We had to spend $70 on pizzas for everyone because I ran out of time by then. What should have cost about $20 wound up well over $100.
I called Contadina today to tell them what happened, and they offered to send coupons good for the 2 cans I UPC codes for. I really wasn't expecting them to reimburse me for the sausage and pizzas, but they could have taken my word for the 6 cans, considering I had 2 valid UPC codes and could have sent receipts for the rest.
I'm going to give the coupons to a local family shelter and just go with another brand. Not because they didn't reimburse me for the 6 cans, but because this is at least the 4th time this has happened. I used to think I either accidentally added salt or the garlic was making it taste salty. Now I know that they don't catch the cans that go out right before they recalibrate the salter, from what I was told, so this is going to continue to be a problem. The woman was really nice, though. She even called back a little later and said she was adding another coupon for my troubles.
