    A great self-paying PPO.

I pay about $500 a month for myself, DH and our son, and employer pays close to $1,900. (that's with dental)

I pay $10 a mo. I am 47. This is just for myself. Also comes with life and accident policies.

      Medical Plan- $10 mo.
      Dental $8.33mo.+ Calstar $2.25 mo..[/B] About $20 a mo. total for medical, vision & dental, accident & life policy + life-flight.

      Dental & Vision- $8.33 mo.

MEDICAL FLIGHT TRANSPORT- $2.25 per mo/$50 year. FAMILY COVERAGE

A recent decision by Calif Governor Brown resulted in anyone who works for our Company (or similar companies) can be doubly covered without medical billing issues. That wasnt intentional just a wonderful result of such. If you know half way how to work it, it's easy to avoid medical billing issues. We are paid by two entities (the county for our 20 hrs per wk, then our second employer is a vendor of the State Regional Center, 18 hrs wk). We are on site working doing the same job all hours of our shift so it was ruled "as one job". With the ACA requiring large employers to offer medical benefits for 30+ hours employeees, we get a bit of a smorgasboard of medical plans, dental plans and other coverages mixed in.

To add an 85% employer funded Kaiser HMO plan to act as a CATASTROPHIC PLAN w/Chiropractic coverage, alongside my $10 a mo. medical plan, it would cost me $58 a mo.

It would cost me about $90 a month for coverage of Medical, Vision, Dental. Our Indian Clinic employed a lady who handles insurance and specializes in where to send patients referred by the Doctors. So in the middle of a doc visit, it's not uncommon to be sent out to her office then brought right back to Doc upon how to proceed.
For the Kaiser $45/1500 deductible HMO plan, Employer pays 85%. I am paying off decades ago medical bills via Credit Card Bills. After our $14,000 CC bill is paid, I will do Kaiser HMO
      .

Being Native American, I have a level of FREE Medical & Dental thru my Indian Health Clinic which I don't use because my medical and dental plans thru work cover costs.

Definitely! The monthly premium is lower and we get to pay OOP costs with pre-tax dollars.

We currently spend about $600/month on OOP expenses not covered by our current plan because the provider is out of network. That bill is going up to $800/month.

Under the new plan, it will still be out of network so we'll still be paying OOP except with pre-tax dollars. The HSA limit is $6,750 but we'll save the 25% tax on that money on top of the lower monthly premium.

Preventive care (mammogram, colonoscopy, annual physical, etc.) is still covered 100%. And once we meet the deductible, coverage is 100% on everything but prescriptions.

Almost embarrassed to say. 100% paid Kaiser medical and Delta Dental for wife and myself from past employer. $20. co-payment. Newer employee's no longer are offered this package.

We have a HRA for medical. The base price for family is $500/month. We received credits for a Well Being initiative and for being non-smokers, reducing the month premium to $365. The company funds the HRA with $800 and employees can earn up to $300 more for participating in another wellness program.

Annual deductible for In Network is $1,000 individual/$2,000 family.

After deductible, co-insurance is 80% covered 20% member responsibility.

Maximum Out of Pocket is $3,750 individual/$7,500 family.

I believe this is the third year being on this plan. It has been a major adjustment. Prior to this plan, we paid our monthly premium, a co-pay and then was covered at 100%!

My colonoscopy in 2012 cost me a $100 copay. Received one in 2015 and really took it up the rear (pun intended) for that one. I don't know the exact cost but it was over 1,000 out of pocket and ended up wiping out the entire company contribution to our HRA.

My family of 6 is on an HRA plan. The cost is $270.06 per month.

Annual deductible is $5000 for the family (company puts $2000 a year in HRA spending account that accrues if you don't use it)

after deductible is met its 90/10 until $10,000 is reached. Then its no deductible for the rest of the year.

                    some exams are free
                    1. annual wellness
                    2. mammograms
                    3. well woman exam
                    4. immunizations
                    5. pediatric exams

We have been on a plan like this for 8 years or so, and love the low monthly cost.

Also my younger two are on the state medical card as a backup, so when we are in the 10% zone we just turn those bills into the state to cover. I avoid using the medical card as we like to use our long time family physican, but its a nice backup if needed. (as the kids were adopted from foster care)

Jared what kind of union dues do you pay such that the union can provide health insurance? Is none of the expense covered by the employer?

well my dues are 62.00 a month. I really hope this doesnt turn into a union bashing session. I have been there over 15 years. Without doubt its the best job ive ever had. I will just leave it at that. I dont want to hijack the thread.

There would be no denigrating unions by me. I'm just not familiar with a union providing health insurance. Bargaining for it, yes, but providing it? No, I've never heard of that.

Tricare because husband is active duty military. No premiums. No copay if we see military doctors or are referred. No deductible in most cases.

I was quite annoyed the other day when my military doctor suggested a common test, and I said I'd prefer a less common one. She said it isn't cost effective. Well what if I don't mind paying out of pocket for my own health care! I don't think cost should be a factor in how a doctor decides what they suggest to me for my health care. Give me choices and options and let me decide what I'm willing to pay. Sorry...rant over.

I also found doctor behind the times with their knowledge of some recent medical information. I feel I get what I pay for sometimes...ugh!
