Match 100% of the first 4%. Immediately vested.

Current employer: They match dollar for dollar up to the first 5% of my salary. I contribute 12% total, and they match the first 5%, for a total of 17%. Vested immediately. I didn't get to join the plan til I'd been there 2 years. It's only a PT job so it's growing veeery slowly, but still, I'm lucky to have a PT job with any kind of retirement plan.

Last employer: I had to put in 4% to get the full match, which was 7% in my age category, vested immediately. They matched more the older you got. People over 50 were required to contribute 2% and the company would put in 11%. I hated that they gave more to older people. I felt like I was getting a raw deal just for being young.

The employer before that put in 12%, vested immediately, and the employee didn't have to put in a dime! Very limited investment options, though, and they have a rule that says you can't roll it over, so I still have money in that account. The plan rules will not let me roll it into an IRA or my current workplace plan.
