The age of termination for custodial accounts depends on the state. You can find a table here: http://www.finaid.org/savings/ugma.phtml Once you hit that age, the account is yours. (In fact it was always yours, but your father had responsibility for managing it for you.) How you get to the funds depends on the bank or brokerage where the account is located. Contact them to get more info.
Be aware that you're likely responsible for taxes on the earnings in the account.
