    I think they should.

On the other hand, I (normally when not preg am a size 10-12 at 5 ft 8) had trouble getting into a window seat when I was preg with twins at 7 months flying to a family funeral. A very nice man switched with me--he had the bulkhead aisle seat. Said he was a retired OBGYN and didn't want them squeezed out because he wanted to stay retired and not deliver them in the air! I would not have been happy to pay for 2 seats.

At 6 ft tall, and a frequent size 10 - I don't fit in the seats!

Though I need the one in front of me moved, not beside..

I'm 6'4 235 and don't even begin to fit in the seats. Whenever the guy in front of my decides to try to recline, kiss my knees good-bye, and then I get the dirty look back like I'm infringing on him because he feels my knees through the seat.

I'm all for people who take up excessive width having to pay more, sans pregnant women, but at the same time I think airlines should give a little space between seats, front to back.

Yeah, they should. I fit fine because I am 5'4". I can't imagine someone larger or taller than me fitting well.

You want to be fair to everyone? Let the airlines start charging by the pound. That would be fair. Instead of check in, you do a weigh in. Like the post office. You and your bags. So much per pound per mile. Might lead to an over use of the restrooms before check in.
