The purpose of this study is to better understand the factors that affect breastfeeding choices. Although the results of this study will not directly benefit you in any way, the study will contribute to our knowledge of the factors that affect breastfeeding choices.
Please complete these questions only if you are at least 18 years old, are able to read and understand English, are in your third trimester of pregnancy (or are returning four months after the birth of your baby), and have access to a computer. If you agree to participate in this study, your responses will be used for research purposes only, and your individual responses will not be examined. All responses will be kept confidential for a period of seven years, after which the data will be destroyed. You will be providing an email address only for the purpose of follow-up at the second data point collection time and so that you can receive your compensation. You will be asked to provide your email address in a separate window which will not be linked to your responses. You may withdraw from this study at any time without penalty.
Your agreement to this document will allow you to access the questions. These questions should take approximately 15 to 20 minutes to answer. Please be aware that once you begin the survey, you must complete it in one session, as you will not be able to return after exiting. You will be asked to complete these forms one time during your third trimester, and again during your fourth month after the birth of your baby (postpartum). You will be sent an email reminder when it is time to complete the second set of questions. There is no right or wrong answer, and you are asked to answer as truthfully as possible. Please answer each question and do not leave any questions blank. Choose the answer that best fits for you. Please do not accept help from anyone else when answering the questions and do not allow any one to see your answers.
When you have completed the questions, you will be asked to proceed to the next screen, where you will be asked to provide your email address for follow up at the second data point collection time and so you may be sent your $10.00 gift certificate to Amazon.com®. It is unlikely that you will experience any risk or discomfort associated with this study.
