I'd love to know how you did it....It asked me for a membership number. How do you get around that if you are not a Sam's Club member?

Although I am a member, when I applied I didn't put in my number, the space for the number was listed as optional and I just didn't fill it in.

The membership number was optional, so I simply left it blank. I hope you can get it to work for you.

I can't get to the form. I click the link on the Crest page but no form is on it. Just a bunch of ads for Sam's Club. This freebie is really bugging me.

Under the ad for crest it says: The dentist checks its, Crest Pro-Health Protects it.

Under that it says Click here in blue. Click that and it take you to the form to fill out and the member number is optional there.

I've clicked that very link 7 or 8 times. It just takes me to a page that lists all the departments to shop in. There is no form or link to a form on there. Why that damn Sam's Club just doesn't want me to have that toothpaste! LOL!!!!!!!!!!!!!!!!

I can't get it either. The only thing I can think of is that we must have some type of pop-up blocker that is preventing us from getting the form. I am going to try it on a different computer later to see if that makes a difference.
