It's taken several days...but I finally received an email from them, starts with:

I filled out the g/b and when I hit submit, it just took me back to the g/b and with empty fields. Did I do it right? LOL!!

I got the same thing as rockinrobin...

I just emailed the company instead of submitting my request via the form. It seems that the companies are more generous when you contact them directly.
