My advice is to just fill out the first set of questions. Then the top of the page says that your sample will be sent in x# of weeks. After that, close the page... don't fill out the rest; it is just so they will know what offers to send to your e-mail account. (Also, I use an e-mail address that can be disposable for all such things.) Just a couple of suggestions.
