The form is found stuck on the hasbro games. good thru dec. 24, 2007 Watch for sales.. At target currently $7 for some of the games listed.. with $3 off in rebate makes them $4 each!

there are two different rebates: over $30 visa card and over $40 visa card.

for each game you purchase you get from $2 to $5 a game. no upc code required. only a cash register receipt and the form has a space to write the upc code for each game purchased. A few examples of the games:
